repeat Prescriptions
Has the doctor issued you with a repeat prescription? If so you can order your next prescription items in any one of the following ways:
PLEASE NOTE:
1.ONLY items already authorised by your doctor as repeat prescription items can be ordered by these means. If you need anything else, you must make an appointment to see your doctor
2. The Practice closes some afternoons during the year for essential training. Please bear this in mind when ordering your medication
fill in a repeat prescription order form (available at reception) or tick the items you need on the white, tear off slip from you last prescrition and post it into the box on the wall just by the reception window
faxing your request to 01928 590212 using the white tear-off slip from your last prescription, with the necessary items ticked OR by handwriting/typing your request - just make sure you clearly identify your name, address, date of birth, and exact name and dosage of each item you require.

PHARMACY COLLECTION SERVICE
ask your pharmacy to place your order and collect your prescriptions on your behalf
WE NO LONGER ACCEPT TELEPHONE REQUESTS FOR REPEAT PRESCRIPTIONS
Provided we pick up your request before 4pm, your prescription should be ready for collection after 2pm two working days later. Make sure you put in your repeat request in good time so that you don't run out of medication!!
Please ensure you leave yourself sufficient time to renew your medication. We routinely review all our patients on repeat medication, so from time to time your doctor may request for you to make a routine appointment to review your medication(s) is still working effectively. Please check your prescription to see if the doctor has requested to see you before issuing further prescriptions. The practice takes regular advice from a community pharmacist and changes its prescribing regime in line with current best practice.
Local pharmacies now provide a prescription order, collection and delivery service free of charge. If you are interested in this then please speak to your local pharmacist.
