Welcome to ‘Care for older people’ page designed to give you, your family or carer information on what services, benefits etc. are available within the Halton area.

At Grove House Practice and Heath Road we believe the health of our older patients is highly important. We offer the below services if they are required, simply speak to a member of our Reception team who will book you a suitable appointment:

  • Medication Review with our Clinical Pharmacist or GP.
  • Health Check with one of our Nursing Team
  • Memory / Dementia Assessment with a GP
  • Falls with a GP
  • Advanced Care Planning (this includes preferred place of care and end of Life Preferences) and would be an appointment with a GP.

Are you concerned about Dementia?

Our GPs would be happy to help you with any concerns you have around dementia, we can even give you an assessment to ensure that if you are concerned you may have or develop dementia.

Benefits Available To Over 75sBenefits Available to over 75’s

There are many benefits for people over 75, such as free prescriptions, eye tests, winter fuel payments and free TV licence.

Free Eye test to your home:

Did you know that if you aged 60 and over and are unable to leave your home, you can have a free eye test service come out to you? The Mobile Opticians specialise in providing eye examinations to patients in their own homes and those on eligible benefits are also entitled to FREE NHS SPECTACLES. They provide an efficient, friendly, personal and professional service with opticians who carry out a thorough eye examination using the latest technology, which also includes Glaucoma testing. Winter Fuel Payment:

Free TV License for over 75’s:

After your 75th birthday, you can get a free TV Licence. It covers you as well as anyone you live with. Please remember that you don’t automatically get an over 75 TV Licence on your 75th   birthday. You have to apply for one.

Falls Prevention

Bridgewater Community Healthcare run a Falls Prevention service within the Halton area, a referral can be made via Health Professionals, Social Services, Voluntary Agencies, Fire/Ambulance or self-referrals.

The objectives of the service are to deliver primary prevention advice, assess further falls risks, and provide interventions and to refer on to other services as appropriate.
